- Doctors Office Visits – $75 per visit, 10 visits per year
- Hospital Outpatient Visits - Up to $250 per visit, $1,000 per year
- Ambulance Services – $200 per sickness or accident
- Hospital Confinement – $1,000, $500 or $100 up to one year
